Appointment Status due to COVID-19

Due to the ongoing concern over COVID-19 and desire for safety, we are still limiting potential exposures for all our patients and staff. Reno Surgical Associates wants to ensure that patients who do need to be seen are able to be evaluated with appropriate precautions in place, and that those who do not wish to come to the office can make other arrangements.

At this time, we are able to proceed with TeleHealth appointments for some established patients, but not for new patients. If you select option 4, we will check with your surgeon to see if this is appropriate.

If you are scheduled to see your surgeon for your FIRST postoperative appointment since surgery, please simply keep this appointment. You do not need to continue with this form.

PLEASE READ CAREFULLY, AS THE OPTIONS HAVE CHANGED.

Kindly help us by choosing one of the following:

1. NEW OR ESTABLISHED PATIENT WITH URGENT ISSUE – KEEP APOINTMENT: I am having an active issue that I feel needs to be evaluated now despite current COVID-19 concerns (for example, new cancer diagnosis, symptomatic hernia or gallbladder, change in symptoms or exam, or other urgent problem). I will complete the Phreesia online check-in process for screening through the link sent to my text or email. If I am not able to do this online, I will call no later than the day before my appointment to undergo phone screening.2. NEW PATIENT WITH NON-URGENT ISSUE – CANCEL APPOINTMENT FOR NOW: I am still more concerned about COVID-19 exposure than I am about my condition for which I was referred, and am choosing to postpone my appointment at this time. I will call back when I feel I am ready to come in.3. ESTABLISHED PATIENT - KEEP APPOINTMENT FOR NECESSARY EXAM OR TREATMENT: I am scheduled, or due, for an appointment and have ongoing need for exam, wound care, and/or a drain. I will complete the Phreesia online check-in process for screening through the link sent to my text or email. If I am not able to do this online, I will call no later than the day before my appointment to undergo phone screening.4. ESTABLISHED PATIENT - CANCEL IN-PERSON APPOINTMENT BUT WOULD LIKE A TELEHEALTH APPOINTMENT INSTEAD IF ABLE: I am scheduled, or due, for a routine appointment, but am not having any active issues at this time. I would like to have a telehealth appointment instead, if appropriate.5. ESTABLISHED PATIENT - CANCEL APPOINTMENT WITHOUT NEED FOR RESCHEDULING: I am scheduled, or due, for a routine follow up appointment, but am not having any active issues at this time. To streamline care and reduce unnecessary exposures, I would prefer to continue follow up with either my PCP or, if applicable, oncologist, and will call back as needed.6. NONE OF THE ABOVE: None of the above categories apply. Please ask someone to call me to discuss my specific circumstance.
